Introduction
Private medical services provided by Dr Sera Shoukru, sole trader, trading as The Marylebone Practice

Please read these Terms & Conditions carefully before your appointment. They are intended to explain how our private medical service operates, what fees may apply, and the responsibilities of both the Practice and patients.

About the practice

  1. The Marylebone Practice is the trading name for the private general practice services provided by Dr Sera Shoukru, a sole trader. The Practice provides private healthcare services and charges fees for all consultations, services, investigations, treatments and related costs.
  2. Patients will be advised that payment is required for private consultations, treatment, investigations and any costs incurred through The Marylebone Practice.
  3. Payment for a doctor consultation is required in full in accordance with the Practice fee policy. A consultation may be refused or rescheduled if payment has not been received.

Fees, Investigations, and Prescriptions

  1. Additional investigations, treatments, letters, reports, referrals, prescriptions or administrative services may attract additional charges. Where reasonably practicable, these charges will be explained before the relevant service is provided.
  2. You are not obliged to proceed with any additional investigation or treatment recommended by a doctor. If you decide not to proceed, you acknowledge that this may affect diagnosis, treatment planning or clinical recommendations. The doctor will record the discussion and any decision not to proceed in your clinical notes.
  3. Any prescription issued by the Practice is provided on a private fee-paying basis. The Marylebone Practice and its doctors are not responsible for pharmacy dispensing decisions, pharmacy charges, medication availability, or the cost of any medication.
  4. Prescriptions are issued for the named patient only. If a prescription is lost, a duplicate may be issued at the doctor’s discretion and may incur an additional charge. A doctor may decline to issue or re-issue a prescription where it is not clinically appropriate.
  5. Recommendations made to your NHS GP or another healthcare provider are advisory. Your NHS GP or other provider remains responsible for deciding whether they can accept, implement, prescribe, investigate or refer in line with their own clinical judgement, policies and commissioning arrangements.
  6. The Marylebone Practice and the doctors providing services are not responsible for inaccuracies, delays or clinical risk arising from incomplete, inaccurate or misleading information supplied by you or by a third party on your behalf.

Doctors and Clinical Responsibility

  1. Where you consult with an independently contracted doctor, that doctor is responsible for the clinical care they personally provide. Dr Sera Shoukru is not responsible for the acts or omissions of independently contracted doctors, except where the law provides otherwise.

Appointments, Cancellations, and Service Limits

  1. The Practice may charge a cancellation fee where an appointment is cancelled with less than 24 hours’ notice or where a patient does not attend. The cancellation fee may be up to the full cost of the booked appointment or service, together with any reasonable costs already incurred by the Practice.
  2. Our staff are entitled to work in an environment free from violent, threatening, discriminatory, aggressive or abusive behaviour. Such behaviour will not be tolerated.
  3. The Practice may decline, pause or withdraw access to services where a patient behaves violently, threateningly or abusively; refuses or repeatedly fails to make payment; or requires emergency, specialist or hospital-level care beyond the Practice’s scope or capacity. Patients requiring urgent or emergency care should contact NHS 111, 999, or attend an appropriate urgent or emergency care service.

Confidentiality, Records, and Complaints

  1. We will not disclose your confidential information to third parties without your consent, except where disclosure is required or permitted by law, including where there is a serious risk of harm to you or others, safeguarding concerns, or a court or regulatory requirement.
  2. Patients are entitled to access their health records in accordance with applicable data protection legislation, including the UK GDPR and the Data Protection Act 2018.
  3. If you have any concerns about the service you have received, please contact the Practice as soon as reasonably possible. A copy of the Practice Complaints Procedure is available on request from any member of staff.
